Title: Senior Care Initiatives at JDH: NICHE/Delirium Protocols/Post-Acute Care

Learning Objectives: Participants will (be able to):

1. Discuss the current goals to improve quality being implemented by the NICHE Program. 2. Recognize the current deficiencies that exist with regards to sharing information with post-acute care locations. 3. Be able to list at least 2 initiatives which are being implemented at john Dempsey Hospital to improve communication with post-acute care locations.

Target Audience: The Department of Medicine faculty, housestaff and students, though many topics will be of interest to housestaff, students and other faculty members within the University of CT Schools of Medicine and Dental Medicine..

Accreditation: The University of Connecticut School of Medicine is accredited by the Accreditation Council for Continuing Medical Education (ACCME) to provide continuing medical education for physicians.
