Title: Department of Medicine: Quality Grand Rounds Part 2 – Readmission Morbidity and Mortality Lessons Learned

Learning Objectives: Participants will (be able to):

1. Learn about John Dempsey Hospital’s readmission rates and associated financial penalties. 2. Discuss how our monthly Readmission Review Conference helps to educate our residents, faculty, and other providers: leads to the development of quality improvement projects; and leads to individualized care plans for patients with high healthcare utilization.

Target Audience: The Department of Medicine faculty, housestaff and students, though many topics will be of interest to housestaff, students and other faculty members within the University of CT Schools of Medicine and Dental Medicine..

Accreditation: The University of Connecticut School of Medicine is accredited by the Accreditation Council for Continuing Medical Education (ACCME) to provide continuing medical education for physicians.

The University of Connecticut School of Medicine designates this live activity for a maximum of 1 AMA PRA Category 1 Credit(s)™. Physicians should claim only the credit commensurate with the extent of their participation in the activity.
